• I the undersigned have read and accept the BKB Internet ADSL2+ Supply Agreement, Terms and Conditions, Acceptable Use Policy, Director’s Guarantee and Indemnity, and Spam Policy (located at http://www.bkb.net.au/).
• I have read and understood the information about BKB Internet’s handling of information about me (personal information).
• I agree that by signing of this form, BKB Internet Pty Ltd is under no obligation to accept my application until they are satisfied that all of the information I have provided is true and correct.
• If BKB Internet considers it relevant to assessing my/our application for commercial credit, I/we agree to BKB Internet obtaining from a credit reporting agency a credit report containing personal credit information about me/us in relation to commercial credit provided by BKB Internet.
• I/we agree to BKB Internet obtaining personal information about me/us from other credit providers, whose names I/we may have provided for BKB Internet or that may be named in a credit report, for the purpose of assessing my/our application for commercial credit made to BKB Internet.
• I/we agree that BKB Internet may obtain a consumer credit report about me/us from a credit reporting agency for the purpose of collecting overdue payments relating to commercial credit owed by me/us.

I/we understand that:

• Veda Advantage will give BKB Internet information about me for that purpose.
• BKB Internet will give information to Veda Advantage, and that Veda Advantage will hold that information on their systems and use it to provide their credit reporting service.
• When other Veda Advantage customers use the Veda Advantage credit reporting service, Veda Advantage may give the information to those customers.
• BKB Internet may use Veda Advantage’s credit reporting services in the future for purposes related to the provision of credit to me. This may include using Veda Advantage’s monitoring services to receive updates if any of the information held about me changes.
• If I/we default in my/our payment obligations to BKB Internet, information about that default may be given to Veda Advantage, and Veda Advantage may give information about my default to other Veda Advantage customers.

I/we acknowledge that this Application includes a Guarantee and Indemnity and this Application is subject to the Guarantee and Indemnity being granted in favour of BKB Internet (unless BKB Internet agrees otherwise). I/We declare that this account is wholly for business purposes.

Finally I/we agree to pay BKB Internet Pty Ltd the fees above as a part of this application and understand that a full refund will be made if this application is rejected.

Important: You should not agree to this declaration unless this account is wholly or predominantly for business purposes. By agreeing to this application you may lose your protection under the Consumer Credit Code.
